Well I thing that you need to take a bird eye view on you diet plan and try to found real mistake which become hurdle in achieving desire goal. Im trying to solve your problem by writing some tips.

Regular work out is a key to reduce weight,
Cardio exercise like walk, running, bicycle daily for 20 to 30 mints,
Use fresh fruits,
Eat raw and boiled vegetables mostly instead of healthy and fat foods,
Drink plenty of 8 to 10 glasses of water per day,
Avoid smoking if you smoke,
