How should we Name to make Numrology Chart

Usually in india actual name is over NAME + Father Name + Surname we write in papers but mostly people known by Name + Surname & Friends they call by Just Name.
So pls tel me which name should we write

1-NAME + Father Name + Surname
2- Name + Surname
3- Just Name

In drawing up a chart  one should use the full name at birth,  example first middle last  for it adds insight into other areas of the chart .  but charting should also  done on varations to the name  being that many people do not include their whole name in daily life situations.
