Why do miscarriages happen?

I was just wondering about miscarriages today. My office assistant's daughter fell in her 7th month and lost the child 10 months ago. She was pregnant again at 4mths. Yest she had a miscarriage:( Why do they happen? is it because of some physical strain or more because the fetus isnt formed well or some chromosomal defect? Any way such things can be avoided in the future for her? I feel so sorry for the poor girl.

Thanks Airgon. Have even told the girl to speak to a doc before thinking ahead of another child.
